We will develop and test a ‘clinical prediction rule’ to identify patients with long term conditions who are at risk of becoming high users of unscheduled or urgent care.

We will gain an understanding of factors affecting access to urgent care which are common to different conditions and those which are disease specific. We will use these findings to identify general and disease specific measures which reduce unmet needs and reduce unscheduled care use.

We will directly address the help needed by many patients with long term conditions who are identified as depressed in routine screening.

We will develop an intervention to use in conjunction with the clinical prediction rule which will be user informed and consistent with the aim to improve and promote self management in long term conditions.
