We intend that the impact of the research will improve the recognition of psychosocial problems in patients with long term conditions. Those in need of urgent or unscheduled care will be identified promptly and, if appropriate, be offered effective, beneficial treatment. 

 

We plan the following outputs from the various stages of the programme

  1. A clinical prediction rule system to identify patients with long-term conditions who are at risk of becoming frequent users of unscheduled or urgent care.

  2. A detailed description and  full understanding, of who accesses unscheduled care, why they do this, what they experience, and what happens to them. 

  3. Development of  a credible, effective low intensity intervention to be used in conjunction with the clinical prediction rule to prevent frequent unscheduled or urgent care.
